Not really sure what kind of answer you’re expecting. Only Tesla and maybe the NHTSA would really know that.

Could be a specific worker. Could be a batch of parts. Could be a combination of the two. Could be a power issue in the tools. Could be a shitload of things most people aren’t privy to, nor something required to publish to the public.
