As a cyclist I feel far safer on a roadway than I do on a bike path (which has the problem of exiting where drivers aren't expecting anything) or bike lane (which are a) dangerous and b) usually not usable because of all the glass litter).
I guess it depends on where you are. For me it's bike lane > (roadway, bike path) > sidewalk. (Actually sidewalk is a no-go unless I'm walking my bike.) Well, except in the winter or right after the snow melts off (now) because the bike lanes are full of snow (winter) or sand (now). But the rest of the year we don't have broken glass (really? I'm sorry).

And people in the Netherlands would presumably rank bike lane far ahead of any of the other options. They usually have a physical barrier between the bike lane and both sidewalk and road, and they often have their own traffic lights (worked into the overall light pattern of course).
