Starting when?Petro-Canada has published their prices.
Depending on the province, it is $12, $15, or $20 per hour, billed by the minute.
There doesn't appear to be a surcharge for higher power.

Arbour said there are two theories behind the vandalism.
One is that the vandals were after the copper at the ends of the cables, but “talking with the technician, it sounds like the copper value is probably 20 or 30 bucks.”
“So it’s hard to fathom somebody cutting the power supply to everything and going through all this trouble, just for that.”
He said the second theory is someone showing their frustration over electric cars.
Arbour added that to “properly grow electric vehicle infrastructure,” you need reliability.
In the news for Ontario, Ivy Charge (hydro one and Ontario power generation) announced to build a charging network.
details are here:
Unfortunately, they haven't updated their site in months and details are lacking. For instance, still no info on pricing.
On the website, it says Huntsville, ON location is now open and that the charging rate for 50KWH is 30 cents a minute (100kwh and 150kwh have their own undetermined price).
There is also a map with locations, some locations now have addresses, not all.
