CA - how to fill reg1000 form to convert white HOV stickers to new ones

I got my original white HOV stickers around end of 2017 and never received a pre-filled application from DMV to get replacement stickers. I read the following DMV article but am having a hard time figuring out how to fill the form correctly to get my white decals upgraded to the new purple stickers.

Also, is it true that if I mail in my application on Jan 1, 2019, I will get the new purple stickers that are valid until Jan 1, 2023 rather than the red ones that are valid until Jan 1, 2022?

The following website makes me think that the replacement decals will be valid until Jan 1, 2022 rather than 2023 for vehicles that were issued a decal in 2017 or 2018 originally.

No. Because you already have one prior to 2019 and between 2017 to 2018.

Sorry. The purple one is for those who did not have one prior to 2019.

Since you already got a white one in 2017, you can renew it for a red one that expires in 1/1/2022 which is a consolation prize of full 3 additional years.

I renewed mine so I can tell you the process and the error I made.
When you fill it out select NEW for that one checkbox. I had selected replacement and it turns out that was the wrong box.

simply request the replacement saying you lost it. they send you new "Red" one. I did it to my wife's car.

I never got one either. Id send it in no matter what. They wont cash your check until its processed. I had mine returned twice to me because I checked the wrong box and forgot to fill in one box.

I would send the form in after people have started to receive the purple decal. This way you should get purple too.

Takes much longer than that. Just keep waiting. Im sure they have a rush from all the model 3 sales

You have to color the edges with a gold marker so it looks like lakers colors
