CVRP $2500 Rebate process

I'm finally getting around to asking for my $2500 rebate using the CVRP form. Maybe I'm just not looking in the right places, but I have a couple of questions to which I don't see answers online. Perhaps those of you who have done this can help.

1. Who do I put down as the Tesla contact? What phone number? What title?
I just put "Sales" and gave their general sales number. I'm doubting that will be a problem. Anybody know different?
2. They ask for "A complete copy of your executed and signed vehicle lease or purchase agreement." I don't think I have such a thing.
The documents page on My Tesla has my Purchase Agreement, which looks good enough except that it isn't signed. I don't remember getting any sort of hardcopy, but maybe I did and misplaced it. They explicitly say "Please include all pages and sections with signature and date of purchase/lease visible." So what have people done successfully?

I gave them the final contract that is on "my tesla" showing full payment. I copied the DMV paperwork that they affix to the passenger side windshield to show the official title transfer and registration in my name, i.e., the temporary registration. I used my delivery specialist as the Tesla contact, although I doubt that it matters.

I suppose that you could wait until you receive the pink slip in the mail and use a photocopy of that. But I used the temporary registration info along with the payment page on "My Tesla" and my request sailed through unfettered.

I know this is an old thread, but I just want to mention that for #2 above, you could submit the "Purchase Agreement" downloaded from Tesla account page but you have to sign and date all the pages and re-scan them. I didn't read the CVRP form as carefully as OP so I submitted the form as is without any signature/date. Two months later, I got an email telling me to sign the forms. So I did it, re-submitted them and got the approval and "we will let you know when we mail our your check" email the next day. I still have not received the check yet a month later... not sure how long it will take.
 
CVRP staff members review applications in the order that supporting documents are received. Our average times to first review are shown below. These estimates were last updated: 6/6/2018.

  • Applications not selected for income verification: 55 calendar days.
  • Applications selected for income verification: 47 calendar days.

Did you submit anything else besides the unsigned Purchase Agreement? I submitted the Purchase Agreement unsigned but also included the signed (by both parties) Delivery Declaration that I signed when I picked up the 3. On the CVRP website, there is a pdf showing all the required documents to submit (and samples) including a section specific to Tesla:

For the Tesla purchase agreement is shows - Acceptable Tesla document combinations:

Option 1: One document from Column A [Retail Installment Contract or Tesla Lease Agreement or Tesla Conditional Sale Contract and Security Agreement] or

Option 2: All documents from Column B (electronically signed/time stamped) [Motor Vehicle Purchase Agreement: Final Price Sheet and Motor Vehicle Purchase Agreement: Vehicle Configuration Sheet] or

Option 3: All documents from Column B (not signed; not time stamped) AND one signed document from Column C [Delivery Declaration and Due Bill or Motor Vehicle Purchase Agreement: Terms & Conditions]

I went with Option 3 as I didn't have any from #1 or signed from #2.
 
When we took delivery, we didn't receive any paper at all. 100% paper free. They said everything would be online later. But the only thing online was the Purchase Agreement pdf without any signature. I picked Option 2 but kind of missed the (electronically signed/time stamped) part.

Two months later, I got "Your Application is Incomplete" email saying that
Thank you for applying for a rebate with the Clean Vehicle Rebate Project (CVRP). While reviewing your supporting documents for application (application #), we noticed the following issue(s):

Purchase/Lease Agreement:
  • A final signature was not included on the copy of your purchase/lease agreement. Please submit a complete copy of your agreement (including all pages and sections) with your signature visible. Your signature may be on the reverse side of the agreement.
So I just printed them out, signed and dated them. Scanned the pages and resubmit them.
